2011 Year-End Forms for Congregations

Thanks in advance for interpreting our work and mission to your people and for encouraging them to grow in their giving to your congregation and to your synod. After your congregational meeting please fill out Your Congregation’s Plan for Mission Support and the Congregational Leadership Form. NOTE: The two forms are due back in the synod office (or submitted online) by January 31, 2012.

The Annual Congregational Report Forms A & C can be completed and submitted on the ELCA website.

Annual Report to the Bishop for Ordained and Lay Rostered Ministers

It is important that you fill out and submit your report. Bishop David B. Zellmer looks forward to hearing from you and will personally read your report.

The form may be filled out electronically on the ELCA website.

To complete the Annual Rostered Leader’s Report to the Bishop:

Be certain that the latest version of Formatta Filler is installed on your computer.
Click on the appropriate form link, matching both your roster category (lay or clergy) and your roster status.
Fill in the information requested.

There are two options for returning the form to the bishop. You can:

Send it to the synod office as an e-mail attachment, to cmccormick@sdsynod.org.
Print a copy and mail it to the South Dakota Synod office at:

Ministry Site Profile (MSP) Form for Congregations in Call Process

Congregations entering the call process will find the Ministry Site Profile (MSP) Forms at the ELCA website. Completing the MSP and contacting the synod office are the first steps to beginning the call process. For a guide to completing the MSP form, click here.

Rostered Leader Profile (RLP) Form For Rostered Leaders

“Call Process” refers to the process by which rostered leaders — ordained or lay — move from one call to another in the ELCA. Information and forms pertaining to the call process may be found at the ELCA web site.

The Rostered Leader Profile (RLP) form is the new Mobility papers that must be submitted electronically. For a guide to the RLP form, please click here.
